Required Documents for Enrollment:

• Proof of Required Immunizations (State of Arizona)
• Social Security Card
• Census Number or Certificate of Indian Blood
• Birth Certificate (Copy)
• Current Transcript/Report Card (Preferred)

Required Age for Enrollment: A child is eligible for admission if he/she is five (5) years of age prior to September 30th of the current school year.
